All forms must be complete with parent’s signature and date. You will have to present BIRTH CERTIFICATE OR OTHER FORM OF IDENTITY PROOF, IMMUNIZATION AND HEALTH INFO. FORM HAS TO FILL OUT BY PHYSICIAN before your child may be registered. Please keep all phone numbers and contacts current. This is always available should an emergency arise.
HOURS OF OPERATION
We open at 8:00 am and close promptly at 6:00 pm. All children must be picked up on time. We simply ask that you not be late. If you run into a problem, please contact us and let we know who will pick up your child and how long it will be. We will inform your child to keep him/her from worrying. If the child is not picked up on time, of course, if you are late someone will remain with your child. There will be a charge of $5.00 for late fee for the first 15 minutes and a $1.00 per minute charge following for each child.
TUITION AND FEES
Tuition is charged weekly and payable by the Monday of each week. A tuition week is Monday through Friday. Daycare will be closed on holidays listed on our annual calendar. Your tuition remains the same regardless of holidays, vacations, illness, snow days, or other acts of nature.
In order to reserve a space, we require a DEPOSIT of ONE week’s tuition. It is your child space as long as you pay tuition, even if your child is not in the daycare for any reason.
DEPOSIT must be paid in advance which will be applied to your child care fee at the time of withdrawal when only written notice is given or you will forfeit your deposit. We will not pay you interest on your deposit, and we may commingle with your deposit with our general funds.
Daycare reserves the right to adjust tuition at anytime. At least two weeks' notice will be given before a rate increase is put into effect.
The parent will be ask to remove your child from daycare when you do not cooperate with our policy (such as non-payment, refuses to deposit, behavior of child, etc.)
Payment is considered past due if not paid in full by 6:00P.M. Tuesday of each week; at which time a $15 late tuition fee will be charged weekly until tuition and fees are paid.
If a check is returned by the bank, there will be a $20.00 handling charge from our banking institution, $10.00 a day late fee until tuition is paid in full in cash. If a pattern of returned checks of 2 or more times cash payment is expected there after and only cash. Childcare will not be provided until all fees are paid.
Daycare will also close for two weeks during the year for vacations. This fee will be required to pay after six months of service. You will be responsible for making other childcare arrangements. A two weeks notice is required for vacation periods for both family and caregiver.
When it is necessary that a child be absent for any reason, it is requested that you call us by 9:00 am to report the absence. No reduction in tuition is allowed because of absences due to illness or otherwise.
Please dress your child in clothes that are comfortable, washable, and appropriate for a day of play and learning. Any clothing that may be taken off and misplaced (hats, sweaters, mittens, coats, etc.) should be clearly labeled with your child’s name. An extra set of clothes-pants, shirt, socks, diapers, and underwear for use in the event of toileting accidents, spills, etc…
(Please clearly mark each piece items with child’s name) this extra set for your child used when needed, the clothes he/she is wearing will be returned to you in the plastic bag.
Please make certain that your child’s extra clothes are changed seasonally. I ask that all children take their shoes off at the door. If your child is not yet tying their own shoes, please provide slip-on shoes or shoes with Velcro. Daycare is not responsible for any unlabeled clothing or shoes lost.
USE OF PICTURES: Parent(s) hereby gives permission for use of picture(s) or video of their child participating in Daycare activities for publicity purposes.
CARE/SUPERVISION: At least one member of our staff trained in CPR and Certified in First Aid is present at all times in the event of an emergency. Standard First Aid procedures (including washing the area involved and applying a bandage will be used for minor scrapes and cuts. Children in diapers may need cream applied occasionally. In all situations, we will follow instructions of the poison control center or physician when providing First Aid procedures and/or administering medications. Unfortunately, accidents can occur that may require medical attention. Parents are responsible for all medical costs for their child through their own personal insurance.
HEALTH POLICY: Children are happier and they learn better when they feel alert and energetic. In order to maintain this standard of health, parents are not to bring children to the Daycare with any of the following conditions: Temperature above 101, unexplained rashes, non-food related diarrhea, nausea, vomiting, severe cough or severe congestion. Conjunctivitis (pink eye), flu-like symptoms, and any contagious conditions (i.e. sore throat, measles, head lice, mumps, chicken-pox …etc … all families involved will be notified.
Medication will be administered only if there is signed permission form from a licensed physician. If your child becomes ill during care, you will be asked to pick up your child immediately. If you cannot be reached, we will call one of the emergency numbers you have listed. You child may return to childcare when symptoms have subsided. If your child is home more than 3 days, you must bring a singed physician’s statement when returning to the program. Parent(s) agrees that they will inform the Daycare within 24 hours or the next business day after if any member of the immediate household has developed any reportable communicable disease, except for life threatening diseases, which must be reported immediately.
Note: There is no bath is given during daycare hours in order of childcare’s insurance.
Daycare provided proper nutrition food for kid one year old and over. Children enrolled full-time are served a breakfast, nutritious lunch and snack each day. No soda or candy will be served. A physician’s statement is necessary when providing special diets. Food for special (i.e. birthdays, holidays) may be brought in by parent’s prior arrangements. Please do not send your child to us with extra snacks, drinks, candy or gum. Children are taught proper manners at the table.
Formulas ____ baby food ____ brought by parent. Infants will be fed according to parent’s instructions. Please update and notify caregiver of any changes in feeding schedules, formulas, and additional foods. Milk will be furnished all infants who no longer need a special formula. Breast-fed infants need to have an adequate supply of expressed milk in bottles. All bottles must label with your child’s name.
We are using safety equipment for children but no matter how hard we try to ensure the safety of your child, minor accidents may occur. Children learn by testing their abilities. They should be allowed to participate in activities appropriate for their development even though these activities may result in some minor injuries. Scratches and scrapes minor bump or bruise are inevitable when children play. The injuries will be treated with antiseptic and if necessary, and Band-Aid. With anything more serious, we will telephone you immediately, and take appropriate actions.
Two weeks written notice is required for withdrawal of your child for any reason, or parents will be charged for that period.
Daycare will be closed on Holidays (see Year 2012).
